Health Reports - 05/12/2009

The following food service establishment inspection reports were provided by the Environmental Health Division of the Rockdale County Health Department and the Newton County Health Department. Restaurants that score from 90 to 100 get a letter grade A; those that rate 80-89 get a B; a tally of 70-79 is a C; and below 70 is a U, or unacceptable. A restaurant scoring 70 or above is allowed to operate if there are no critical violations. If a facility is rated U on consecutive inspections, it must close and the owner must prove that it can be operated safely before reopening is allowed.

· Mamies' Inc. 84-B, 7121 U.S. Highway 278, Covington. Date: April 6

Violations: Sanitizer weak at three-compartment sink. Food not being cold held or hot held properly. Not using time controls properly. Cups in eating area not protected. Employee using vegetable sink to wash and prep raw chicken.

· Burger King, 87-B, 2230 Salem Road, Conyers. Date: April 7

Violations: Person in charge could not communicate or convey management awareness policy. No written procedure of file or using time as a public health control.

· Island Jerk, 68-U, 875 Flat Shoals Road, Suite 130, Conyers. Date: April 7

Violations: Person in charge not demonstrating knowledge of duties or food safety. Employee put on gloves without first washing hands. Bare hand contact with ready-to-eat food. Potentially hazardous food not held at the proper temperature. Employee wearing jewelry while in process of preparing food. Damaged pan lids still in use. Shelves with oil spilled on them. No paper towels in restroom. Damaged ceiling tile in employee restroom. Damaged wall and missing cove base at floor wall juncture in store room.

· Island Jerk - Recheck, 100-A, 875 Flat Shoals Road, Suite 130, Conyers. Date: April 14

· Dominos, 78-C, 3537 Ga. Highway 20, Conyers. Date: April 8

Violations: Observed employee making pizza without using gloves. No adequate handwash sinks. Need handwash sink in dishwash area. Current inspection report not posted. Not in compliance with Georgia Smoke Free Act. In-use scoop not properly stored. Damaged gasket on make line reach-in. Toilet facilities not properly supplied, toilet paper not supplied for dispenser. Restroom has hole in wall by sink.

· Dominos - Recheck, 95-A, 3537 Ga. Highway 20, Conyers. Date: April 16

Violations: Person in charge could not convey the required requirements when to report, exclude, restrict an employee from working. Need additional hands sink in dishwash area. Restroom wall damaged.

· Honey Creek Wing Cafe, 96-A, 1033 Honey Creek Road, Conyers. Date: April 8

Violations: Employee observed wearing watch while prepping food. Debris build-up on outside of soda dispenser.

· Rockdale Health Care Center, 100-A, 1510 Renaissance Drive, Conyers. Date: April 8

· Smiley's, 44-U, 2001 Main St., Porterdale. Date: April 8

Violations: Employee did not wash hands before putting on gloves to work with food after she had been cleaning. Dish washer did not wash hands between handling dirty dishes then handling clean ones. Many employees never washed hands at hand sink during inspection. Employees touching cooked sweet potatoes barehanded, also touching vegetables while cutting barehanded. Employee drinks in coolers above food items. Employee eating cookies while in food prep area. Hand sink blocked by big piece of equipment in kitchen. Storing sanitizer bucket in hand sink at front area. Front hand sink did not have any hand towels. Badly dented cans in dry storage area. No sanitizer at three-compartment sink or dishwasher. Not hot holding food at proper temperature. Not date marking food that they hold more than 24 hours. Sanitizer buckets not labeled. Rust and mold in top of ice machine. No digital thermometer. No sign at buffet for people to get new plates. Utensils at mop sink not protected from splash up. Food next to hand sink in back not protected from splash up. Need to store utensils 6 inches off floor. Not storing pans, bowls and other utensils inverted. Employees cooking did not wear hair restraints. Stacking clean cups and utensils wet. Styrofoam cups not dispensed properly. Employees wearing gloves to perform different duties without changing gloves. No vegetable sink. No C1 test strips. No covered trash can in women's bathroom or handicap accessible bathroom. Need covered trash cans. Wet mop stored in bucket. Cove molding loose behind three-compartment sink. Back screen door does not seal properly at top or bottom.

· Waffle House, 63-U, 12985 Brown Bridge Road, Covington. Date: April 8

Violations: Employees had bare-hand contact with cooked bacon and waffle. Employees' drinks stored next to food items on shelves. Employee had gloves o n going between utensils with raw meat juice on handles to touching ready-to-eat food without washing hands between duties. Found mold on inside top of ice machine. Mold in slicer. Improper level of bleach in sanitizer buckets. Wet wiping cloths sitting on counter and around sanitizer buckets. Stacking utensils wet at three-compartment sink. Plates not inverted at bar. Employee with gloves on going between raw juices and ready-to-eat food. Spray wand down in sink. Employee's purse and shirt next to food items.
