Restaurants that score from 90 to 100 get a letter grade A; those that rate 80-89 get a B; a tally of 70-79 is a C; and below 70 is a U, or unacceptable. A restaurant scoring 70 or above is allowed to operate if there are no critical violations. Violations: No designated person in charge present. No hand washing observed. No soap available at handwash sink in kitchen. Raw chicken stored over ready-to-eat food. Mold-like substance in ice maker. Food product stored in refrigerator that was not date marked as required. Observed out-of-date product. Unlabeled food product in refrigerator. No handwash signs at handwash sinks. Damaged door gasket seals on cooler. Deteriorating fryer baskets still in use. Water heater leaking. Flies in kitchen.

Violations: Mixer bowl at stand mixer has rust. Three-compartment sink not sealed properly to wall. Ice build up in walk-in cooler.

Violations: Employee handling ready-to-eat food needs to not handle utensils with raw meat juices then touch ready-to-eat foods. Employee handling raw foods then touching clean plates and bowls without washing hands first. Employee slid cooked bacon off spatula onto plate with no gloves on. Sanitizer too weak in bucket. Employee not sanitizing tables properly. Can opener blade dirty. Ice machine's interior not clean. Cold hold foods not at proper temperature. Employee drinks and cigarettes stored on food prep surfaces. Sanitizer too strong in bucket. Single-use utensils stored next to chemicals. Food containers not labeled. No accessible hand sink at dish-washing area. Cleaning utensils in use at chicken prep area only once per day. Sanitizer bucket stored next to hot holding units.
