Health Reports - 1/20/2009

The following food service establishment inspection reports were provided by the Environmental Health Division of the Rockdale County Health Department and the Newton County Health Department. Restaurants that score from 90 to 100 get a letter grade A; those that rate 80-89 get a B; a tally of 70-79 is a C; and below 70 is a U, or unacceptable. A restaurant scoring 70 or above is allowed to operate if there are no critical violations. If a facility is rated U on consecutive inspections, it must close and the owner must prove that it can be operated safely before reopening is allowed.

· Waffle House No. 899, 87-B, 2060 Sigman Road, Conyers, Date: Dec. 3

Violations: No paper towels available at rear handwash sink. Mold-like substance in ice maker. Condensate leak in pie cooler.

· Subway, 90-A, 1620 F Dogwood Drive, Conyers, Date: Dec. 3

Violations: Paper towel dispenser with no paper towels at rear handwash sink. Several tables not cleaned during inspection. Wet utensils stored stacked on one another. Commode lid in men's restroom damaged. Dust accumulated on exhaust vents in restrooms.

· Pippin's, 94-A, 4580 Ga. Highway 20, Conyers, Date: Dec. 5

Violations: Mold-like substance in ice maker. Dumpster placed on base ground. Damaged floor in kitchen.

· Daddy's Bar & Grill, 53-U, 1381 Iris Drive, Conyers, Date: Dec. 5

Violations: No designated person in charge present. No hand washing observed. No soap available at handwash sink in kitchen. Raw chicken stored over ready-to-eat food. Mold-like substance in ice maker. Food product stored in refrigerator that was not date marked as required. Observed out-of-date product. Unlabeled food product in refrigerator. No handwash signs at handwash sinks. Damaged door gasket seals on cooler. Deteriorating fryer baskets still in use. Water heater leaking. Flies in kitchen.

· Mickey Dee's, 96-A, 6147 U.S. Highway 278, Covington. Date: Dec. 9

Violations: Chemicals stored in food prep area.

· R.L. Clements Middle School, 95-A, 66 Jack Neely Road, Covington. Date: Dec. 10

Violations: Mixer bowl at stand mixer has rust. Three-compartment sink not sealed properly to wall. Ice build up in walk-in cooler.

· Waffle House No. 625, 6137 U.S. Highway 278, Covington. Date: Dec. 10

Violations: Employee handling ready-to-eat food needs to not handle utensils with raw meat juices then touch ready-to-eat foods. Employee handling raw foods then touching clean plates and bowls without washing hands first. Employee slid cooked bacon off spatula onto plate with no gloves on. Sanitizer too weak in bucket. Employee not sanitizing tables properly. Can opener blade dirty. Ice machine's interior not clean. Cold hold foods not at proper temperature. Employee drinks and cigarettes stored on food prep surfaces. Sanitizer too strong in bucket. Single-use utensils stored next to chemicals. Food containers not labeled. Plates not stored properly at bar area. Plastic containers stored wet. Drawers under grill area dirty. Sugar and pepper stored in old syrup bottles. Vegetable sink not installed per code. Door at prep cooler does not close properly. Hose end down in flood plain of mop sink. Need backflow device between spigot and hose. Spray wand down in sink in front area. No self-closing door at bathroom. Make sure you clean the fan cover in the walk-in cooler and reach-in coolers regularly. Need handwashing sign in men's bathroom.

· Church's Fried Chicken, 68-U, 3275 U.S. Highway 278, Covington. Date: Dec. 12

Violations: Employee cleaning then handling okra to put in fryer without washing hands. Employee put on gloves without washing hands then handled food. Employee handling raw chicken then handling cooked chicken without washing hands.. Employee coming in to work did not wash her hands before performing work duties. No accessible hand sink at dish-washing area. Cleaning utensils in use at chicken prep area only once per day. Sanitizer bucket stored next to hot holding units. Filter power stored on top of and beside flour. No label on sugar or flour storage container. Employee needs more adequate hair restraint. Wet wiping cloths in food prep area. Employee using whisk to prepare potatoes then storing in soap solution. Sugar scoop handle down in sugar. Frying basket broken. Hot holding pan broken. Walk-in cooler door not sealing properly. Cracked lid on filter powder. Top of hot holding unit dirty. Top underside of hot holding unit dirty. Walk-in cooler doors dirty. Air curtains dirty. Gaskets dirty on walk-in cooler door. Hose end down in mop sink. Spray wand down in flood plain of three-compartment sink. Mop sink faucet leaking. Tile cracked. Walk-in cooler floor cracked. Employee food in walk-in cooler.
