Health Reports 11/11/2008

The following food service establishment inspection reports were provided by the Environmental Health Division of the Rockdale County Health Department and the Newton County Health Department. Restaurants that score from 90 to 100 get a letter grade A; those that rate 80-89 get a B; a tally of 70-79 is a C; and below 70 is a U, or unacceptable. A restaurant scoring 70 or above is allowed to operate if there are no critical violations. If a facility is rated U on consecutive inspections, it must close and the owner must prove that it can be operated safely before reopening is allowed.

· Nagoya Japanese Steakhouse, 95-A, 1375 Iris Drive, Conyers, Date: Oct. 1

Violations: Employee observed preparing food with no hair restraint. Previous inspection not posted. In-use utensils stored in product. Single-use container reused for other product.

· Hong Kong King, 36-U, 6238 Turner Lake Road, Covington, Date: Oct. 3

Violations: Employee did not wash hands between handling dirty and clean dishes. Employee drink in reach-in cooler. Raw chicken in hand sink - use hand sink for washing hands only. Frozen pork sitting on top of frozen chicken. Eggs stored next to produce. Bacon next to chicken in walk-in cooler. Chicken uncovered in walk in cooler. No sanitizer in dishwasher. Dirty can opener. Mold in ice maker. Eggs held out of temperature in prep area. Multiple items out of temperature at buffet. Rice was out of temperature at sushi bar. Chicken improperly cooled. Chicken was held out of temperature without time documentation. Sanitizer in bucket too strong. Can of rubbing alcohol sitting out above ice maker. Used soap only to clean cutting board - need to wash, rinse and sanitize. Wet wiping cloths in food prep areas. Prepped chicken stored under handsink. Oil stored on floor. Food stored in cans. Containers of food stacked together - need barrier between containers. Rice in dry goods not sealed. Bowls used to scoop cooked rice, sugar, chips. Utensils stored wet. Equipment in kitchen not working. Knives, spoons, forks stored in wrong direction. Shelving dirty beneath prep table. Vent-a-hood accumulation grease. Floor in walk-ins dirty. Sprayer stored beneath rim of sink. Employee restroom dirty. Walls by dishwasher area dirty. Threshold at back door opening missing. Fly tape hanging over food prep area. Improper thawing of ribs.

· Hong Kong King - Recheck, 66-U, 6238 Turner Lake Road, Covington. Date: Oct. 9

Violations: Employees did not wash hands at any point during inspection. Raw chicken stored above egg roll. Raw eggs next to produce. Raw chicken next to shrimp. Chicken uncovered in walk-in cooler. Dirty can opener. Sanitizer buckets: one had no sanitizer, one had too much sanitizer. Oil stored on floor. Food on floor in walk-in freezer. Sanitizer bucket stored on floor. Wet wiping cloths stored out of sanitizer. Utensils stored wet. Walk-in cooler ceiling has build up. Sprayer held below rim of sink at dish washing area. Floors dirty in kitchen.

Violations: Employee observed touching ready-to-eat food with bare hand. Door gasket seals of reach-in freezer damaged.

· Zaxby's, 91-A, 1871 Ga. Highway 138, Conyers, Date: Oct. 6

Violations: Food items in walk-in freezer not covered as required. Mold-like substance in ice maker. Food items in walk-in cooler not date marked as required. Fryer baskets deteriorating. Door gasket seals of reach-in cooler damaged.

· Captain's Table, 96-A, 1815 Ga. Highway 138, Conyers, Date: Oct. 7

Violations: Mold-like substance in ice maker.

· Honey Creek Wing Cafe, 97-A, 1033 Honey Creek Road, Conyers, Date: Oct. 7

Violations: Fryer baskets deteriorating. Floors around equipment need replacing.

· Kentucky Fried Chicken, 80-B, 1675 Ga. Highway 138, Conyers, Date: Oct. 7

Violations: Employees were not washing hands as required. Employee drinks stored with ready-to-eat food. Food items stored on floor of walk-in freezer. Inspection report not posted at drive-thru as required. Soiled utensils stored with clean utensils. Fryer baskets seals on reach-in coolers. Soiled door gasket seals on reach-in cooler. Inside of microwave ovens soiled. Observed damaged lexan pans stored with good utensils. Outside of most equipment soiled. Dumpster doors were not closed. Dumpster did not have a drain plug. Floors and walls of entire kitchen area soiled, need thorough cleaning.

· Applebee's Neighborhood Grill, 78-C, 1448 Dogwood Drive, Conyers, Date: Oct. 8

Violations: Mold-like substance in ice maker. Potentially hazardous food not held at the proper temperature. Employee with unapproved jewelry preparing food. Dispensing utensils stored with handle in products. Wet utensils stored stacked on one another. Damaged door gasket seals on reach-in cooler. Soiled door gasket seals on coolers.

· House of Lin, 69-U, 2445 Salem Road, Conyers, Date: Oct. 8

Violations: Two employees observed washing hands in vegetable sinks and not handwash sink. Employee observed not washing hands before cooking food after wiping down table. Raw chicken in walk-in cooler stored over raw shrimp. Pre-cooked chicken stored covered in reach-in prep table cold hold unit with white cloth towel covering food. Opened can of sauce stored in original container. In-use wiping cloths not stored in sanitizing solution knife stored in between equipment. Outside of rice warmer dirty with debris. Fryer basket damaged.

· House of Lin - Recheck, 98-A, Date: Nov. 14

Violations: Wet utensils stored stacked on one another. Soiled knife stored with clean utensils. Employee used glove for different tasks. Damaged door gasket seals on reach-in freezer. Soiled door gasket seals on reach-in cooler.

· Waffle House, 93-A, 1174 West Ave., Conyers, Date: Oct. 8

Violations: Mold-like substance in ice maker. Dish machine not reaching sanitizing temperature.

· Project Adventure, 96-A, 396 Elks Club Road, Covington. Date: Oct. 10

Violations: Insecticides stored with chemicals.
