Health Reports - 12/9/2008

The following food service establishment inspection reports were provided by the Environmental Health Division of the Rockdale County Health Department and the Newton County Health Department. Restaurants that score from 90 to 100 get a letter grade A; those that rate 80-89 get a B; a tally of 70-79 is a C; and below 70 is a U, or unacceptable. A restaurant scoring 70 or above is allowed to operate if there are no critical violations. If a facility is rated U on consecutive inspections, it must close and the owner must prove that it can be operated safely before reopening is allowed.

· Chili's, 95-A, 1570 Dogwood Drive, Conyers, Date: Nov. 4

Violations: Mold-like substance in ice maker. Soiled can opener blades. Damaged Lexan pans/lids. Damaged door gasket seals. Soiled door gasket seals.

· McDonald's, 81-B, 10701 Covington Bypass Road, Covington, Date: Nov. 4

Violations: Raw eggs stored over ready-to-eat food. Tea nozzle dirty. No hand washing sign at handsink at three-compartment sink and men's bathroom. Utensils stored wet. Dirty under coffee maker at drive-thru. Fan covers dirty in walk-in cooler. Light out at hot holding unit for fries. Plumbing leak under sink next to handsink. Hose end down in mop sink. No hand towels in men's and women's bathroom.

· Starbuck's at Target, 96-A, 2195 Ga. Highway 20, Conyers, Date: Nov. 4

Violations: Handwash sink blocked by coffee urns.

· Jim 'N Nicks Bar-B-Que, 91-A, 2275 Ga. 20, Conyers, Date: Nov. 5

Violations: Mold-like substance in ice maker. Prepared food item stored next to handwash sink. Dispensing utensils handles stored in product. Deteriorating fryer basket used for cooking.

· McDonald's, 49-U, 4147 Salem Road, Covington, Date: Nov. 5

Violations: Employee did not wash hands and put on gloves before handling ready-to-eat food. Employee handling ready-to-eat food with glove on one hand and no glove on other hand. Employee had a personal drink with no lid in food prep area. No soap at hand sink. No hot water at hand sink. Coffee grounds in handwash sink. Wiping cloth left in hand sink. Raw eggs stored next to tea containers. Shredded cheese and tartar sauce stored uncovered in walk-in cooler. Frozen chicken patties and strawberries stored uncovered in walk-in freezer. Old food left down in vegetable prep sink. Coke nozzles dirty at drive-thru and self-serve area. Cheese sitting out at room temperature with no time documentation. Weak sanitizer in sanitizer buckets. No digital thermometer working. Wet wiping cloths out. Sanitizer buckets stored on floor. Bacon container sitting in shredded cheese container. No food inspection report at drive-thru. No hand washing sign in men's bathroom. Single-service use items stored on floor in dry storage area. Soap dispenser broken at hand washing sink. Reach-in freezer hinge broken. Hamburger freezer lid cracked. Ice machine broken. Sanitizer system missing cover, handle and leaking, Storage rack broken in dry storage area. No light covers in dry storage area. Ice build-up on fans in walk-in freezer. Smoke detector missing in men's bathroom. Water standing behind reach-in cooler at front. Dirty under orange juice machine, coffee maker and cream dispenser at front and drive-thru. Trash down in drain at floor at vegetable prep sink. Dirty around and under bagel prep area. Dirty behind stove cook area. Storage area has dirty floors. Mop sink drain clogged and has standing water. Sanitizer system leaking at three-compartment sink. No hand towels in men's and women's bathrooms - must have hand towels with pull handle doors. Trash overflowing at trash can near hand sink. Tile removed above mop sink area. Mops drying on floor.

· McDonald's - Recheck, 92-A, 4147 Salem Road, Covington, Date: Nov. 7

Violations: No cover on ice container. Plastic spoons stored with handle down in cup. Hamburger freezer lid cracked. Dirty above ketchup and mustard dispenser. Hot-hold unit is dusty. Hose down in mop sink. Gap at back door at bottom. Light out at fry hold.

· Remington House, 95-A, 1504 Renaissance Drive, Conyers, Date: Nov. 5

Violations: Mold-like substance in ice maker. Deteriorating fryer basket. Damaged door gasket seal on walk-in cooler.

· Porterdale Elementary School, 93-A, 45 Ram Drive, Covington, Date: Nov. 6

Violations: Slicer dirty on underside. Chopper dirty. Scoop with handle in sugar product. Mesh drain under three-compartment sink not open enough to let water through without backing water up onto floor.

· Royal Palace, 77-C, 4188 U.S. Highway 278, Covington, Date: Nov. 6

Violations: Employee drink in food prep area. Coke nozzles dirty. Vegetable sink dirty. Can opener dirty. Food at prep cooler out of temperature. Sanitizer weak in buckets. Scoop and handle down in ice. Utensils stored wet. Gaskets on walk-in cooler dirty.

· Palmer Stone Elementary, 97-A, 110 Emory St., Oxford, Date: Nov. 7

Violations: Gaskets on walk-in cooler dirty. Ceiling and light fixture in walk-in cooler have build-up. Light has water inside cover. Area at back door and around Dumpsters has some trash accumulation. Hole in wall by electrical switch and by electrical cord in chemical room. Damp mop in mop sink.

· Alcovy High School, 87-B, 14567 Ga. Highway 36, Covington, Date: Nov. 10

Violations: Employee drink stored by utensils. Chopper dirty when taken apart. Coffee urn spout is dirty. Improperly cooling grits. Utensils stored wet. Gasket on walk-in cooler door is taped - need to repair gasket.
